Process improvement & Impact on Clinical Quality & Safety/Patient Care
Dr. Flarity created the first validated ED specific fall risk assessment tool worldwide, setting a benchmark for comparison. During this 1-year study period (April 2011–April 2012), a total sample population of 91,190 patients 18 years and older were included in the study. As Primary Investigator, Dr. Flarity copyrighted the Memorial ED Fall Risk Assessment tool, not for profit or notoriety but so organizations would require permission to use and share data so the tool could be further validated and tabulated. As a servant leader, Dr. Flarity has granted permission to use the tool in 97 additional emergency departments, representing over 4.5 million ED patient visits. The overarching goal of the project was to improve patient safety and eliminate patient falls resulting in serious injury in the ED. An ED-specific tool was designed on the basis of the risk factors consistently identified in the literature: prior fall history, impaired mobility, altered mental status, altered elimination, and the use of sedative medication. The Memorial ED Fall Risk Assessment Tool was validated in two EDs (North campus and Central campus) located within a large urban health care system in Colorado Springs, Colorado. The two EDs have a combined 140,000 annual patient visits. The Memorial ED Fall Risk Assessment Tool appears to be a valid tool for this two-ED hospital system.
Throughput Process Improvement Example:
Emergency departments (EDs) are challenged to provide rapid triage and evaluation to make appropriate patient disposition and timely treatment decisions. Cardiac troponin is the preferred biomarker for evaluation of patients with chest pain. The objective of this before-and-after study was to determine the impact of Point-of-Care (POC) troponin testing on turnaround times, door-to-troponin result times, ED length of stay (LOS) in patients with chest pain, and staff satisfaction with POC testing. After POC implementation, the average door-to-troponin result time significantly decreased from 105 to 51 min (p < 0.000). The average LOS decreased from 290 to 255 min; however, the change was not significant (p =0.082). The majority of nurses (81%) felt that POC testing encouraged communication among patient care team members, and satisfaction was high with 82% of all ED staff members rating their satisfaction as excellent. To increase throughput, turnaround time and emergency department (ED) staff satisfaction, we implemented and evaluated point of care (POC) Toponin in our ED.
